How To

• Ontario Marriage Licence Application form can be obtained by clicking here.
• Fees vary: $80.00 (in Ingersoll) or $130.00 (in London) cash, cheque or debit card.
• Both parties must be 18 years of age,
• You must determine where & when you will marry & make any required reservations or a licence will not be issued.
• Both parties must sign the Application Form & produce identification, such as a birth certificate, a valid driver’s licence, or a valid passport.
• One person can purchase Marriage Licence & can be issued to one of the applicants. Both parties must sign the Marriage Licence application form & ID for both parties must be presented.
• If divorced in Canada, you must produce an original of the Decree Absolute.
• Two (2) witnesses of the marriage are required; Crystal Chapel can supply these witnesses, if required.
• The Ontario Marriage Certificate can be obtained on-line at www.applicationservices.info & will take approximately 2 weeks to receive.
• Certificate can be purchased 12 week after the date of your wedding for $25.00.
• Mail-in application form can be obtained by clicking here.
• Note: mail-in application may take up to 12 to 36 weeks to receive. The on-line application is the fastest.
